rGENERATION Parent Forms

At rGeneration, we believe children are never too young to know God, experience His love, and begin building a faith of their own. Our children's ministry serves children 12 months through 12 years old in safe, fun, age-appropriate environments created especially for them. Our goal is simple: help kids know God while having fun and experiencing Him on their level.

Your child's safety, security, and privacy are important to us. Parents and legal guardians are asked to complete the forms below to help us provide a safe and welcoming rGeneration experience for every child.

Approved Pick-up Authorization Form

Let us know who you authorize to pick up your child from rGeneration. Authorized individuals must be 16 years of age or older, and a copy of a valid government-issued photo ID must be provided for each person listed.

Media Release Form

Please complete the Media Release Form to let us know your preferences regarding the use of photos and/or video of your child during rGeneration services, activities, and events.
